History: Advanced Pain Management was started in 2007 to provide care for the residents of the Phoenix, Glendale and Tempe area. Thanks to our supportive patients, we have been able to grow our practice from a single provider to more than seven who serve over 40,000 patients.
Brian S Page, D.O.
Dr Page graduated from medical school at Okahoma State University. He is board certified in Anesthesiology and Pain Management and trained at the University of Kansas. He has been practicing Pain Manangement in Phoenix for 12 years. He believes in a multi-discipinary approach to pain management. Combining Interventional pain techniques with physical therapy and medication to relieve pain has been very successful. He is trained in the newest techniques such as Kyphoplasty and Spinal Cord Stimulation. We offer tradional techniques such as epidural steroid injections and other nerve blocks too.
Joshua Levy, D.O.
Joshua Levy DO is originally from Plainview, NY. He attended College at SUNY Buffalo with a major in Psychology. In 2001, he earned his medical degree from New York College of Osteopathic Medicine. He completed his Physical Medicine and Rehabilitation training at Nassau University Medical Center in Long Island, NY. He continued on to complete a Sports Medicine and Interventional Spine fellowship at UMDNJ in New Jersey. During this time, he was Assistant Team Physician for both Rutgers University and New Jersey Institute of Technology. Dr. Levy is board certified by both the American Board of Physical Medicine and Rehabilitation and by the American Osteopathic Board of Physical Medicine and Rehabilitation. Dr. Levy brings great enthusiasm to teh practice of medicine. His holistic approach to patients with physical impairments, disabilities and pain gives him a unique perspective that strengthens his commitment to provide his patients with compassion and understanding. Nichole Brown, NP-C
Nichole is a native to sunny Arizona. She graduated from Arizona State University with her Bachelor of Science in Nursing Degree. Working as a nurse for the Mayo Clinic, she attained her Master's in Nursing Family Practitioner Degree from Grand Canyon University. She is a member of the American Academy of Nurse Practitioners and Sigma Theta Tau. In her free time she enjoys hiking, traveling, and dancing.
Heather Chung, NP-C
Heather graduated and received her Bachelor of Science in Nursing from the University of Portland, Oregon in 2006. She decided to move to Arizona to begin her nursing career. She received her Master's Degree as a Family Nurse Practitioner from Arizona State University. She is a member of the American Academy of Nurse Practitioners and is certified as a nurse practitioner. Heather is married with one daughter and two adorable cats, named Ziggy and Dora. Swimming and spending time with her family are some of her hobbies.
Elizabeth Colman, NP-C
Elizabeth is an Arizona native. She received both her Bacherlor's and Master's Degree in Nursing from Arizona State University. Prior to coming to this practice, she worked as a Cardiac nurse at JC Lincoln Hospital for eight years. She is married with three boys. Her hobbies include, off-roading, camping, and sports. Elizabeth is a member of the American Academy of Nurse Practitioners and is certified as a Family Nurse Practitioner.
Nathan Franke, PA-C
Nathan was raised in Nebraska and finished his Bachelor's degree from the University of Nebraska. He furthered his studies and completed his Master's Degree in Exercise Physiology from the University of Georgia. He then practiced as clinical exercise physiologist in cardiac rehabiliation for seven years at St. Vincent's Regional Center in Santa Fe, New Mexico. Nathan furthered his education by earning his Master's in Physician Assistant studies from A.T. Still University. He is a certified by the National Commission on certification of Physician Assistants who specialize in pain management and spine surgery. In his spare time, he enjoys his two loving dogs, biking and traveling.
Leslie Kotsis, NP-C
Leslie was born and raised in Houma, Louisiana and received her Bachelor's of Science in Nursing Degree at Nicholis State University. She attend Wayne State University in Detroit to obtain a Master's of Science in Nursing. Leslie is board certifed by the American Academy of Nurse Practitioners and is also a member of the American Academy of Pain Management, Sigma Theta Tau Honor's society. She currently lives in Gilbert with her husband and two children.
Dave Dorland, RPT
Dave graduated from A.T. University in 1999 with a M.S. in Physical Therapy. My goal is to reduce or eliminate your pain through manual therapy, exercises, and education. I want to empower my patients to learn to move more naturally. We recently added, Cliff Fortunat, Phyical Therapy Assistant to help Dave so we can accomodate our patients with a more flexible schedule and offer more locations to serve them.
Melissa Hillstead, PA-C
Melissa was born and raised in California. She received her Bachelor of Science degree in Biology from Brigham Young University. She received her Master Medical Science degree in Physician Assistant Studies from Midwestern University in Glendale, Arizona. Melissa is currently certified by the National Commission on Certification of Physician Assistants. She is a member of the American Academy of Physician Assistatns and Phi Kappa Phi Honor's Society. In her free time she enjoys traveling, reading and swimming.
Chad Dance, NP-C
Chad was born and raised in Idaho. He received his Bachelors of Nursing from Idaho State University. After graduation he began working in the ICU where he worked for over four years before moving to Arizona. He continued to work in the ICU where he worked for over four years before moving to Arizona. He continued to work in the ICU while he furthered his graduate studies. Chad has been an ICU nurse for nine years now. He graduated with his Masters of Nursing in Family Nurse Practitioner from the University of Phoenix. He is accredited by the American Academy of Nurse Practitioners. In his spare time he loves to play golf, hike, ski, bike, and explore Arizona with his wife and kids.
